Please send all materials in a single envelope/E-Mail with complete
information for each participant on a separate page/form. The
registration form may be photocopied. Group rate does not apply
at the door. All participants in a group must attend the seminar
at the same venue
Refunds: Cancelled registration requests must be received
in writing before the date of your seminar. You may: 1) transfer
to another seminar, 2) request a voucher for a future seminar or
3) receive a refund of your paid tuition less a $20 administrative
fee. ( Refund requests will not be accepted after the date of your
seminar.)
