*Gargoyle Z was stripped of the title when he left the MMP on June 1, 1998. The title was held up in a tournament held at the Championship Fever PPV on June 17. The tournament was won by Blitzkrieg, defeating Diamond King in the finals.
**Diamond King was stripped of the title immediately following his title win on January 30, 1999 as a result of the circumstances under which he won the belt. The vacancy was filled in a ten-man tag team match at the Main Event Madness PPV on February 24, pitting Diamond King, The Ryno, Captain Patrick O'Conner, Gemini Killer and The Ghettoman against Masochrist, Super Cupid, Psychotic Sam, Aries, and Mr. Marvellous. The title was won by Ryno, who pinned Super Cupid in the match.

*"The Canadian Pimp" Ouija Lullabye substitued for Masochrist in the match where the belts changed hands.
**O'Conner & Gralen were stripped of the belts when Gralen attacked O'Conner following a title defense.
***Richards defended the belts on his own in a four way ladder match against Aries, Malice, and Dr. No Limit with the condition that the winner would be able to choose any partner to share the championship. Richards won the match, and chose himself as his the other co-holder of the belts.
****The title was vacated when a "winner takes all" match between Richards and The Joker's Child ended in a double disqualification. The vacancy was filled with a tournament in early 2000, won by Death & Wolverine.
*****Aries awarded his share of the tag team championship to Baital on July 26, 2000 at BattleGround 3 in an effort to force Hades and Baital to learn to co-exist with one another. Officially, as of July 26, all three of Aries, Hades, and Baital are one-time holders of the tag team title.

*The Regulator was stripped of the title when he was fired from the MMP in December 1998. Psychotic Sam and Christian Antamarius were deemed to be the two top contenders to the title at the time and met to fill the vacancy on December 12.
**Sam was stripped of the title when he briefly left the MMP in early 1999. The vacancy was filled at the WrestleStorm '99 in a Break-In Match, which was won by Gemini Killer.
***Because Sam was the reigning universal champion at the time, he was forced to vacate the extreme title. He elected to hand the title over to his stablemate in The Family, Mr. Beumont.
****Because Ryno was the reigning world champion at the time, he was forced to vacate the extreme title. It was held up in a Triple Threat Vanity Match at the Homicidal Tendencies PPV between The Destroyer, Dr. No Limit, and Pot Head where Ryno was the guest referee. The match was won by DNL.
*****Virus agreed to relinquish the championship, effective May 30, in order to secure a world title shot at WrestleFest 2000.

**The Regulator agreed to vacate and retire the title on September 5, 1998, due to a lack of interest in the belt. It was re-instated in a tournament held on March 24, 1999 at the March Mayhem PPV. The tournament was won by Clay Moore, who defeated Virus Richards in the finals.
***After a succesful title defense against The Destroyer at Vindication on September 27, Steel awarded the title to his manager, Adam Kogler, in order to pursue the universal championship.

*Lord Deimos was defeated in a handicap match by Psychotic Sam, Havoc, and Marbels the Cat when he was pinned by Marbels. The Family elected to have Havoc hold the title.
**Neither champion Blitzkrieg nor challenger Pot Head were able to appear for a scheduled October 10 title defense. When this was learned on October 8 the title was vacated and a match signed between Antamarius and Soul Crusher to crown a new champion. Antamarius won the title on October 10.
***Soul Crusher vacated the title prior to a scheduled defense against The Flying Jobber. A battle royal, won by The Ghettoman, was held the following week.
****The Joker's Child was stripped of the title on July 17 for failing to make title defenses against opponents of the minimum required ranking. The vacancy was filled the following week in a match between Wolverine and Big Daddy Cash.
*****After Sam captured the title from Hot Crimson, Brewer offered to vote for Sam in Sam's imaginary King of Spain race in exchange for the title. Sam accepted, and Brewer became the champion.
******Because he was not an active wrestler, Brewer was stripped of the championship on December 29. A one-night tournament was held at the Season's Slayings PPV event to fill the vacancy. Hot Crimson defeated Hassan Abu Saada in the finals of the tournament to claim the title.
