REGISTRATION OF BIRTH

1. Collect the Confirmation of Birth ( Borang Pengesahan Kelahiran) form from Labour Room after delivery and fill the necessary details

2. Registration of Birth is done at the

Jabatan Pendaftaran Negara
Tingkat 1, Wisma Persekutuan
Jalan Bayam, 15200
Kota Bharu.

3. The following documents are required for registration

Mother’s Antenatal Card
Mother’s Identity Card
Father’s Identity Card
Informant’s Identity Card
Marriage Certificate (only for the first child)

4. Registration should be completed within 2 weeks of the birth
